We accept most insurance plans. To find out if we are a provider on your specific plan, either call the customer service number listed on the back of your insurance card, or call our office at (440)967-3937 to speak with Leslie, our insurance specialist. We will be happy to check on your insurance benefits for you. We are participating providers on the following networks. Many of the logos below link to the insurance companies' web sites. This is not a complete list of insurance plans accepted at Dr. Burley & Associates.
While we will gladly bill your insurance carrier for you, please understand that we are estimating the plan benefit that is available to you. Claims denied by or not paid in a timely manner by your insurance carrier will become your responsibility.
